      目的 对比研究看图对话与传统授课方式的在糖尿病健康教育中作用,探索看图对话教育工具在糖尿病健康教育中推广和普及的意义。方法 选取上海交通大学医学院附属瑞金医院内分泌病房患者,随机分为两组,采取传统授课和看图对话两种教育方式进行干预,并采用问卷等形式对干预效果进行调查和统计学分析。结果 看图对话组的糖尿病知识测试分数优于传统授课组;看图对话组出院后在饮食、运动、血糖监测及足部护理等方面的自我管理水平优于传统授课组,在遵医嘱用药方面两组间的差异不存在统计学意义;看图对话组在出院3个月后的空腹血糖值、餐后2h血糖值及糖化血红蛋白值优于传统授课组。结论 糖尿病健康教育中看图对话教育方式在患者的知识掌握、自我管理及血糖控制等方面优于传统授课形式。但部分护理人员及患者的固有教育理念有待转变。看图对话对护理人员的综合素质及硬件设施等要求较高,在一定程度上限制了其应用。对于如何进一步提高三项血糖指标的达标率也有待更长时间的干预和检验。
英文摘要:
      Objective The major objective of this study was to review the current using of the Conversation Map Tools method and traditional lecturing in diabetes education, and to explore the significance of promoting the innovative method. Methods A questionnaire was carried out with patients in Department of Endocrinology and Metabolism of Ruijin Hospital Affiliated to Shanghai Jiao-Tong University School of Medicine. Results The score about diabetic knowledge was higher in the group using the Conversation Map Tools, as well as about self-management of diet, exercise, glucose monitoring and nursing on diabetic foot after discharge, whereas there remained no differences between the two groups about following the prescription. The fast plasma glucose, postprandial plasma glucose and glycated hemoglobin were also better controlled in the group using the Conversation Map Tools than the group using traditional lecturing, yet further studies on how to improve control rates of the three index above were still needed. Conclusions Compared with traditional lecturing, the Conversation Map Tools helped patients have a better understanding of diabetes knowledge, self-management and glucose control, however some of caregivers and patients were still reluctant to wholly embrace this innovative method. Moreover the promotion of this method was in a way limited by the needing of better comprehensive quality of caregivers and better medical resources.
