      本文通过介绍跨理论模型的定义、跨理论模型目前的应用现状及效果,针对行为改变的六个分期(前意向期、意向期、准备期、行动期、维持期和结束期),探讨基于跨理论模型对心血管疾病患者健康行为管理方案构建,旨在为帮助我国护理工作者建立科学有效的心血管疾病护理的健康教育模式提供理论依据。
英文摘要:
      The literature introduced the definition of Transtheoretical Model (the Transtheoretical Model, TTM), as well as the current application and effect.In the light of six stages of behavior change (Precontemplation, Contemplation, Preparation, Action, Maintenance and Termination). The author will explore the construction of health behavior of patients with cardiovascular disease management programmes based on the Transtheoretical Model, aimed at providing theory for establishing scientific and effective health education mode of cardiovascular disease nursing.
