      【】 目的 探讨负压闭式引流中负吸压强对开放性骨折外固定支架钉道感染和骨折愈合的影响。方法 将120例下肢开放性骨折行外固定支架联合VSD治疗的患者随机分为低负压组(<30kPa),中负压组(30-60kPa),高负压组(60-80kPa),每组40 例, 比较三组患者在术后创面愈合时间、骨折愈合时间、1个月钉道感染率、6个月临床疗效等方面差异。结果 中负压组(30-60kPa)患者创面愈合时间、负压引流时间、抗生素使用时间、骨折愈合时间、住院时间均少于低负压(30-60kPa)和高负压组(60-80kPa),且有统计学意义(P<0.05),而低负压和高负压之间无统计学差异(P>0.05)。中负压组、高负压组患者术后1月创面未感染率明显高于低负压且有统计学意义(P<0.05),而中负压和高负压之间无统计学差异(P>0.05)。结论 下肢开放性骨折使用外固定支架联合VSD治疗时,三组比较,中负压组的钉道感染率最低,骨折愈合最快, 值得在临床推广使用。

      【】Objective To investigate the clinical effect of different negative pressure intensity on the treatment of the open fracture patients with external fixation and VSD. Methods 120 patients with open fractures of the lower extremities were randomly divided into <30kPa group, 30-60kPa group, 60-80kPa group, 40 cases in each group, compared with the 3 groups for 1 months and 6 months treatment process, infection rate and fracture healing. Results Moderate negative pressure group in patients with wound healing time, negative pressure drainage time, antibiotic use time, fracture healing time and hospitalization time less than low negative pressure and high negative pressure group, and have statistical significance (P<0.05), but no significant difference between low negative pressure and high negative pressure (P>0.05). About wound no infection and efficiency of 6 months, the moderate negative pressure group and high negative pressure group were significantly higher than those in low negative pressure (P<0.05), but there was no significant difference between moderate negative pressure and high negative pressure (P>0.05). Conclusions In the treatment of lower limb open fracture external fixation and 30-60kPa negative pressure VSD can achieve lower infection rate and faster fracture healing, and it is worth to be popularized.
