| 【】目的 观察健康教育后脑卒中便秘患者的知识及排便情况的改变；了解脑卒中后便秘患者对健康教育的依从性现状；探讨影响依从性的主要因素。方法本研究采用方便取样法对上海某三甲医院神经内科住院部74名脑卒中后便秘患者,住院期间采用自设问卷进行问卷调查和健康教育方法。在出院后两周及一个月再次通过电话随访方式进行问卷调查。通过比较健康教育前后,脑卒中后便秘患者的健康行为方式改变来评价患者的依从性高低,依从性有所下降或不佳者给予依从性影响因素问卷调查。最终有效问卷42例。结果 患者的疾病知识知晓情况、健康行为方式及排便效果情况,在住院期间(健康教育前)、出院后两周及一个月(健康教育后)比较,有统计学意义(P<0.01)；影响依从性的主要因素是患者的健康信念与认知方面。结论 健康教育能帮助患者了解更多关于脑卒中后便秘的防治知识,从而提高患者对便秘的重视及干预便秘的主观能动性,改善了排便效果。另外患者的健康教育依从性中远期有下降趋势。 |

| 【】Objective To observe the knowledge of disease and defecation effect of post-stroke patients with constipation after health education; understand the status of compliance of health education of post-stroke patients with constipation; explore the main factors affecting compliance.Methods Convenience Sampling was used to select 42 cases of the post-stroke patients with constipation in neurology inpatient department of a hospital in Shanghai. We carried out health education for these patients and applied self-formulated questionnaire to survey them. Two weeks and one month after discharged from hospital, these patients were conducted questionnaires again through telephone follow-up. We evaluated the compliance of post-stroke patients with constipation by comparing health behavior changes before and after health education. We applied questionnaire of the influencing factor of compliance to patients whose compliance was dropped or poor. Eventually we completed valid questionnaires of 42 cases. Results During hospitalization (before health education), two weeks and one month after discharged from hospital (after health education), patient"s disease knowledge, health behavior and defecation effect are statistically significant(P<0.01); The main influencing factor of compliance is patient"s health beliefs and cognition. Conclusions Health education can help patients understand more about prevention and treatment of constipation after stroke,thus patients have raised their emphasis on constipation and subjective initiative to intervene in the constipation ,so they have improved defecation effect . Additionally , patient"s health education compliance has been on a downtrend in the middle and long term. |
