      摘要:目的 探讨早期渐进性运动对重症监护室(intensive care unit, ICU)脑出血机械通气患者ICU获得性衰弱(ICU-acquired weakness,ICU-AW)的影响。方法 纳入2015年3月至2017年3月我院ICU收治的148例脑出血机械通气患者为对象,随机均分为两组,各74例。对照组接受常规护理,观察组在对照组基础上接受早期渐进性运动护理。比较两组干预后肌力、巴氏指数评分、功能独立性水平,机械通气时间、呼吸机过渡时间、入住ICU及住院总时间,并发症发生情况。结果,干预后,观察组MRC-score和巴氏指数评分均显著高于对照组(P<0.05);观察组能独立完成沐浴、穿衣、吃饭、修饰、从床转移到椅子、如厕的患者占比均显著高于对照组(P<0.05);观察组机械通气、入住ICU时间及住院总时间均显著短于对照组(P<0.05);观察组ICU-AW发生率及并发症总发生率均显著低于对照组(P<0.05)。结论 早期渐进性运动能够有效改善ICU机械通气脑出血患者神经肌肉功能,降低ICU-AW风险,提升生活自理能力,缩短恢复时间,有较好的临床应用价值。

      Abstract: Objective To investigate the effect of early progressive exercise on the prevention of intensive care unit (ICU) acquired weakness (ICU-AW) in patients with cerebral hemorrhage in ICU. Methods 148 patients with cerebral hemorrhage treated by mechanical ventilation in our hospital from March 2015 to 2017 were enrolled in this study. They were randomly divided into 2 groups with 74 cases in each group. The control group received routine nursing, and the observation group received early progressive exercise nursing based on the control group. The scores of muscle strength, barthel index, functional independence after intervention, duration of mechanical ventilation, ventilator transition, ICU stay and total hospitalization time were compared between the 2 groups. Results After intervention, the score of MRC-score and barthel index in the observation group were significantly higher than those in the control group (P<0.05), the percentages in patients of observation group who could take a shower, dressing, eat, make up and dress up, move for bed from chair and defcation by alone were significantly higher than those in the control group (P<0.05), duration of mechanical ventilation, ICU stay and total hospitalization time in the observation group were significantly shorter than those in the control group (P<0.05), the incidence of ICU-AW and total incidence of complications in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Early progressive exercise can effectively improve nerve and muscle function in ICU mechanical ventilation patients with cerebral hemorrhage patients, reduce the risk of ICU-AW, promote the activities of daily living, shorten the time of recovery, it has good clinical application value.
