      目的 观察实施“一病一品”护理项目前后,脑卒中患者吞咽功能和日常生活活动能力的护理效果,并探讨和构建脑卒中患者“一病一品”项目的护理服务流程。 方法 以240例脑卒中合并吞咽障碍患者为研究对象,根据随机数字表法分成观察组和对照组,每组120例,观察组患者采用“一病一品”护理项目进行标准化流程护理,对照组采用脑卒中常规护理。3周后采用洼田饮水试验评估两组患者吞咽功能。同时采用Barthel生活指数评估患者护理前、护理3周后及3个月后随访日常生活活动能力。结果 3周结束后,两组脑卒中患者吞咽功能正常所占比例均较前增加,且观察组增加更明显,与对照组比较,观察组脑卒中患者吞咽功能正常所占比列为63.33%,而对照组患者吞咽功能正常所占比列为35.0%,两组患者相比差异具有统计学意义(= 19.272,P=0.000);观察组脑卒中患者重度吞咽功能障碍所占 5.8%,P=0.048)。两组患者BI指数评分同护理前比较均增加,但观察组增加更加明显,出院3个月后随访 两组患者BI指数评分同护理前比较仍增加,但观察组增加幅度更大。护理3周后,与对照组相比,观察组BI评分明显高于对照组,差异具有统计学意义(t=12.196,P=0.000);出院3个月随访两组相比差异具有统计学意义(t=10.851,P=0.000)。结论 “一病一品”护理项目,能够显著改善脑卒中患者的吞咽功能,并能提高患者近、远期日常生活活动能力,从而塑造优质护理服务品牌。

      Objective To observe the nursing effect of swallowing function and daily living ability of stroke patients before and after the implementation of "one disease and one product" nursing project, and to explore and construct the nursing service process of "one disease and one product" project for stroke patients. Methods In 240 cases of stroke patients with dysphagia combined as the research object, according to randomly divided into observation group and control group, 120 cases in each group, the observation group were treated by “a disease and a product” nursing mode of the standardization process of nursing, the control group using conventional stroke care. 3 weeks later, the field drinking test was conducted to evaluate the swallowing function of the two groups. Barthel life index was used to evaluate the activities of daily living of patients before, 3 weeks after nursing and 3 months later. Results After 3 weeks of the "one disease and one product" nursing,two groups of stroke patients with swallowing function normal proportion increased, and the observation group increased more significantly, compared with the control group, the observation group of stroke patients with swallowing function in normal proportion is 63.33%, while the control group patients swallowing function for normal proportion is 35%, there was a statistically significant difference between the two groups(t= 19.272, P=0.000);the observation group of stroke patients with severe dysphagia, accounted for 5.8%, while the control group of patients with severe dysphagia, accounted for 13.83%, there was a statistically significant difference between the two groups(t=3.895, P=0.048). Compared with before treatment, the BI index scores of the two groups patients were increased, but the observation group increased more significantly, 3 months after discharge follow-up BI index scores of the two groups compared with before treatment is increased, but the observation group increased even more. After 3 weeks of the nursing, compared with the control group, the BI score of the observation group was significantly higher than the control group,there was a statistically significant difference between the two groups(t=12.196, P=0.000); after 3 months of follow-up compared, there was a statistically significant difference between the two groups (t=10.851, P=0.000). Conclusion "One disease and one product" nursing model can significantly improve the swallowing function of stroke patients, and can improve the patients' daily and long-term activities of daily living, so as to form high-quality nursing service brand.
