      目的 探讨毫米波治疗仪对照射甲状腺手术后伤口愈合度及对患者疼痛评分的影响。方法 收集选取我科2018年1月到11月行甲状腺癌根治术的患者共4485例患者的临床资料,应用国际随机数字法将纳入标准的研究对象随机分为治疗组(n=223)、对照组(n=2242)、治疗组(n=2243),对照组术后予以常规换药及疼痛护理,治疗组在对照组的基础上采用毫米波治疗仪治疗。根据不同的治疗方案,并将治疗组随机分为治疗①组、治疗②组、治疗③组、治疗④组四组,观察各组治疗治疗前后疼痛的程度及舒适度感受效果,包括伤口愈合时间、效果、患者疼痛及舒适度感受。结果 治疗前各组咽痛及慢性痛切口痛人数相比差异无统计学意义(P>0.05),采用不同方式治疗后,治疗组咽痛及慢性痛切口痛人数明显低于对照组,其中治疗②组效果最显著,差异有统计学意义(P<0.05);治疗②组术后住院以及切口愈合等各个指标所需时间均短于其他对照组,且治疗组住院时间及切口愈合时间普遍短于对照组,差异有统计学意义(P<0.05);治疗组视觉模拟评分(VAS)VAS评分与伯格曼舒适度评分(BCS)BCS评分普遍优于对照组,其中治疗②组术后2h、6h、12h、48hVAS评分明显低于其他组别,BCS评分明显高于其他组别,差异有统计学意义(P<0.05)。结论 通过毫米波照射甲状腺术后局部伤口,可有效缓解手术伤口疼痛,促进伤口愈合增加患者舒适度,建议临床选择峡部照射,术后第一天起每日2次的照射方案能够在一定程度上增加治疗收益效果最佳。

      Objective: To discuss the influence of millimeter wave instruments in the wound healing degree and pain scores after the thyroid surgery. Methods:448 patients who underwent radical thyroidectomy from January to November 2018 were randomly divided into control group (n=224) and treatment group (n=224). The control group was given routine dressing change and pain nursing after operation. The treatment group was treated with millimeter wave therapeutic instrument on the basis of the control group. According to different treatment schemes, the treatment group was randomly divided into four groups to observe the degree of pain and comfort before and after treatment. 60 patients underwent the thyroid surgery from January 2016 to January 2018 in our surgery department were selected and assigned to two groups according to the time of returning back to the ward, 30 cases in each group. The control group was treated with the conventional dressing change; the treatment group was treated with the millimeter wave instruments. The therapeutic effect like wound healing rate and time, pain scores and comfort scores were probed. Results: There was no significant difference in the number of pharyngalgia and incision pain before treatment (P > 0.05). After treatment with different methods, the number of pharyngalgia and incision pain in the treatment group was significantly lower than that in the control group, and the effect of the treatment group 2 was the most significant (P < 0.05). The visual analogue score (VAS) and Bergman comfort score (BCS) in the treatment group were generally better than those in the control group. The VAS scores of 2, 6, 12 and 48 hours after operation in the middle treatment group 2 were significantly lower than those of other groups, and the BCS scores were significantly higher than those of other groups (P < 0.05).The Grade A of wound healing rate in the treatment group (96.67%) was significantly higher than control group (83.33%) (P<0.05); the postoperative incision infection rate in the treatment group (3.33%) was significantly lower than control group (16.67%) (P<0.05); compared with control group, the treatment group had the lower pain scores, higher comfort scores, and shorter hospitalization time and wound healing time (P<0.05).Conclusion: TMillimeter wave irradiation of local wounds after thyroidectomy can effectively alleviate the pain of surgical wounds and increase the comfort of patients. It is suggested that isthmus irradiation should be selected clinically, and twice a day from the first day after operation has the best effect.he millimeter wave irradiation on the local wound can relieve the pain, promote the wound healing, reduce the incidence rate of incision infection and poor wound healing, and increase the comfort and therapeutic effect
