      目的:系统评价不同气道湿化方式降低颅脑损伤气管切开患者并发症发生率的效果。 方法:计算机检索Cochrane Library、Web of Science、PubMed、中国知网、维普中文期刊全文数据库和万方数据库公开发表的不同气道湿化方式降低颅脑损伤气管切开患者并发症发生率效果的随机对照试验。检索时间限制在2002年1月1日至2017年12月28日。由2名研究者按照纳入、排除标准筛选文献,提取文献资料。应用Cochrane Handbook质量评价标准对文献进行质量评价,并通过RevMan5.3软件进行Meta分析。 结果:本研究共纳入14篇符合标准的文献,涉及1127例颅脑损伤行气管切开行气道湿化患者。Meta分析结果显示,行持续气道湿化的试验组的气道黏膜损伤出血率显著低于行间断气道湿化的对照组[RR值=0.23,95%CI=[0.17,0.30],P<0.01];行持续气道湿化的试验组的痰痂形成率显著低于行间断气道湿化的对照组[RR值=0.14,95%CI=[0.09,0.21],P<0.01]。 结论:持续气道湿化有利于改善颅脑损伤气管切开行气道湿化患者的呼吸道状况,降低气道黏膜损伤出血、痰痂形成等并发症。

      Objective:To systematically evaluate the effect of different airway humidification methods on reducing the incidence of complications in patients with craniocerebral injury after tracheotomy. Methods:Randomized controlled trials on the effects of different airway humidification methods on the incidence of complications in patients with craniocranial injury after tracheotomy published in China and aboard were retrieved from the Cochrane Library, Web of Science, PubMed, China National Knowledge Infrastructure, VIP Chinese Journal Full-text Database and Wanfang Database. Retrieval time is limited to January 1, 2002 solstice December 28, 2017. Two researchers selected the literature according to the inclusion and exclusion criteria and extracted the literature data. The Cochrane Handbook quality evaluation standard was used to evaluate the literature quality, and the RevMan5.3 software was used for Meta analysis. Results:A total of 14 literatures were included in this study, involving 1127 patients with craniocerebral injury who underwent tracheotomy and airway humidification.. Meta analysis results showed that the bleeding rate of airway mucosa injury in the experimental group undergoing continuous airway humidification was significantly lower than that in the control group undergoing intermittent airway humidification [RR =0.23, 95%CI=[0.17,0.30], P<0.01]. The sputum scab formation rate of the experimental group receiving continuous airway humidification was significantly lower than that of the control group receiving intermittent airway humidification [RR =0.14, 95%CI=[0.09,0.21], P<0.01]. Conclusion:Continuous airway humidification is beneficial to improve the respiratory airway status of patients with craniocerebral injury who underwent tracheal resection and airway humidification, and reduce airway mucosal injury, bleeding, sputum callus formation and other complications.
