      目的 探讨以罗杰斯整体人理论为基础的护理干预措施对围术期患儿焦虑的应用效果。方法 2021年1月—2021年5月,河南省洛阳正骨医院小儿骨伤科收治80例符合入选标准的患儿,采用随机数字表法分为对照组和试验组各40例。对照组常规护理,观察组在常规护理基础上,以罗杰斯整体人理论为基础进行围术期干预,记录术前访视后(T0),离开准备间时(T1),麻醉诱导时(T2),出恢复室时(T3)及术后24小时随访(T4)共5个时间点的心率(HR)、平均动脉压(MAP)。并依次采用儿童情绪适应量表学前版(CEAS-P)、改良耶鲁术前焦虑量表(mYPAS)、诱导期合作量表(ICC)、儿童苏醒期躁动评分(PAED)和儿童情绪适应量表学前版(CEAS-P)对5个时间点的患儿进行评估,记录其评分。结果 观察组CEAS-P(术前)、CEAS-P(随访)评分明显高于对照组,差异具有统计学意义(p﹤0.001);两组患儿CEAS-P(随访)评分明显高于CEAS-P(术前),差异具有统计学意义(p﹤0.001);观察组mYPAS、ICC及PAED评分明显低于对照组,差异具有统计学意义(p﹤0.001);重复测量方差分析,判断观察组各个时间点的护理干预随着时间的变化对患儿心率、平均动脉压的影响,组间×时间的交互作用有统计学意义(p﹤0.001,p﹤0.001)结论 以罗杰斯整体人理论为基础的围手术期护理干预,可明显改善患儿的焦虑情绪,为探寻适合我国医疗环境的儿童焦虑情绪管理方式提供帮助。

      Objective To explore the effect of nursing intervention on perioperative anxiety of children based on Rogers holistic person theory. Method From January 2021 to May 2021, 80 children who met the selection criteria were admitted to the Pediatric Orthopedics Department of Luoyang Orthopedic Hospital, Henan Province, and were divided into control group and 40 cases each by randomized digital table,The control group received routine nursing, and the observation group received perioperative intervention based on Rogers' holistic theory on the basis of routine nursing, the heart rate and mean arterial pressure were recorded at 5 time points including pre-operation visit (T0), leaving preparation room (T1), induction of anesthesia (T2), exit of recovery room (T3) and 24 hours post-operation follow-up (T4). The scores were recorded using the Preschool Emotional Adaptation Scale (CEAS-P), the modified Yale Preoperative Anxiety Scale (mYPAS), the Induction Cooperation Scale (ICC), the Child awakening agitation score (PAED) and the Preschool Emotional Adaptation Scale (CEAS-P). Result The scores of CEAS-P (preoperative) and CEAS-P (follow-up) in the observation group were significantly higher than those in the control group (P < 0.001). The scores of CEAS-P (preoperative) and CEAS-P (follow-up) in the observation group were significantly higher than those in the control group (P < 0.001), The scores of MYPAS, ICC and PEAD in the observation group were significantly lower than those in the control group (P < 0.001), Repeated measurement analysis of variance to judge the effect of nursing intervention at each time point of the observation group on children's heart rate and mean arterial pressure over time, the interaction between groups × time was statistically significant (P <0.001, P <0.001). Conclusion Perioperative nursing intervention based on Rogers' holistic theory can significantly improve children's anxiety and provide help for exploring the management mode of children's anxiety suitable for China's medical environment.
