      摘要:目的 采用Meta分析方法评价重组人表皮生长因子(rEGF)对压疮疗效的影响。方法 计算机检索PubMed、Embase、Cochrane Library、Web of Science、CNKI、WanFang Data、CBM 和 VIP等数据库,收集有关重组人表皮生长因子对压疮疗效影响的随机对照试验,检索时限为建库至2021年4月,由2名研究员独立筛选文献、提取资料并评价文献质量,最后用RevMan 5.3进行Meta分析。结果 最终纳入14个RCT,共896例患者。Meta分析结果显示,与常规治疗相比,rEGF可提高压疮创面愈合率 [OR=3.82, 95% CI(2.85,5.11 ),P<0.00001],缩短压疮创面愈合时间 [MD = -8.15,95% CI( -12.45,-3.86),P=0.0002]。与常规治疗相比,重组人表皮生长因子可增加肉芽组织中羟脯氨酸含量并降低Ⅰ/Ⅲ型胶原比例,从而促进创面修复。亚组分析结果显示,不同治疗时间rEGF组治愈率较高,差异有统计学意义[OR=4.32;95% CI(3.10,6.03 );P<0.00001];结论 对压疮有较好的治疗作用,但仍需开展更多高质量研究予以验证。

      Abstract: Objective: To evaluate the effect of recombinant human epidermal growth factor (rEGF) on pressure ulcers by Meta analysis.Methods: The databases of PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang Data, CBM and VIP were searched. RCTs on the effect of recombinant human epidermal growth factor on pressure ulcers were collected.The retrieval time was from the establishment of the database to April 2021. Two researchers independently screened the literature, extracted the data, and evaluated the literature quality. Finally, Meta-analysis was performed with RevMan 5.3.Results: A total of 14 RCTs were included, including 896 patients.Meta-analysis results showed that compared with conventional treatment, rEGF could improve the wound healing rate of pressure ulcers [OR=3.82, 95%CI (2.85, 5.11),P<0.00001] and shorten the wound healing time of pressure ulcers [MD = -8.15, 95%CI (-12.45, -3.86)].P = 0.0002).Compared with conventional treatment, recombinant human epidermal growth factor can increase the content of hydroxyproline in granulation tissue and decrease the ratio of Ⅰ/Ⅲ collagen para-type, thus promoting wound repair.The results of subgroup analysis showed that the cure rate of RegF group with different treatment time was higher, and the difference was statistically significant [OR=4.32; 95%CI (3.10, 6.03); P<0.00001].Conclusion :It has a good therapeutic effect on pressure ulcers, but it still needs to be verified by more high-quality studies.
