| 目的 分析上海市松江区车墩镇社区卫生服务中心中药制剂一号蛇伤散治疗门诊带状疱疹患者的疗效及优势，以及蛇伤散治疗中，哪些基线信息会影响治疗效果。方法 研究对象为2018年2月至2020年3月期间在上海市松江区车墩镇社区卫生服务中心接受一号蛇伤散治疗的诊疗信息完整的212例带状疱疹患者(男性93例，女性119例），年龄（60.38±14.49）岁。结果 单纯采用蛇伤散治疗的带状疱疹患者，平均止疱时长为5.27天，结痂时长为7.0天，脱痂时长为10.68天，疼痛明显缓解时长为6.53天，疼痛完全消失时长为11.23天。在进行基线不齐校正后，单纯采用蛇伤散治疗者在疼痛明显缓解时间、疼痛完全消失时间上显著优于蛇伤散合并其他用药者。单纯采用蛇伤散治疗者，女性、有过敏史、就诊时基础症状，是有统计学意义的负性疗效影响因素；蛇伤散合并其他用药者，年龄、无病史、有过敏史、就诊时基础症状，是有统计学意义的负性疗效影响因素。结论 车墩社区中药制剂一号蛇伤散对治疗带状疱疹有良好的效果，且在有效缓解疼痛上，既优于蛇伤散合并用药，也优于单纯西药治疗。不同治疗方案中，患者的基线情况会对治疗效果产生不同影响，临床医师可提前将可能的影响告知患者，以用好循证证据，改善治疗结局。但以上结果和结论均基于小样本量的真实世界观察性研究，后期有待通过严密的研究设计，进行前瞻性对照试验，来进一步证实其疗效优势。 |

| Objective: This study analyzed the effectiveness and advantages of Chedun “No.1 Sheshangsan” with the aspect of treating shingles. The Chedun “No.1 Sheshangsan” is made by Chedun Community Health Service Center in Songjiang Area of Shanghai. This study also explored which factors among baseline information are associated with its treatment effect.
Methods: 212 Patients with shingles visiting Chedun Community Health Service Center in Songjiang Area of Shanghai from February, 2018 to March, 2020 were collected (Male=93, female=119,Age=（60.38±14.49）years old). They all accepted Chedun “No.1 Sheshangsan”therapy.
Results: The average duration of ceasing blister among patients who only accepted Chedun “No.1 Sheshangsan” is 5.27 days, the average time of incrustation is 7.0 days, the average length of decrustation is 10.68 days, the time of pain obviously relieving is 6.53 days, and the time of pain completely disappears is 11.23 days. After the correction of baseline irregularity, the patients treated only with Sheshangsan had much less pain obvious relief time and pain complete disappearance time than those treated with Sheshangsan combined with other drugs. For patients treated only with Sheshangsan, female, the history of allergy and basic symptoms at the time of diagnosis had statistically significant association with negative efficacy. For patients with Sheshangsan combined with other drugs, age, no medical history, history of allergy and basic symptoms at the time of diagnosis were also associated with negative efficacy and the association was statistically significant.
Conclusions: The Chedun “No.1 Sheshangsan” is effective to treat shingles. With the aspect of relieving pain, its effectiveness is better than its combination with other medicine and also better than exclusive western medicine therapy. In different treatment schemes, the baseline condition of patients will have different impact on the treatment effect. Clinicians can inform patients of the potential impact in advance in order to take advantages of evidence-based evidence and improve the treatment outcome. However, since the above results and conclusions are based on small sample size observational studies, the treatment effect of Chedun “No.1 Sheshangsan” needs to be further confirmed by more rigorous study design and prospective randomized controlled trials. |
