目的探讨了失效模型和效应分析方法(Failure mode and effects analysis, FMEA)在核酸的采集、清点、运输和接收等全过程中的应用和有效性。方法便利抽样法,以从2020年11月至2021年4月在某院进行核酸采样的标本为主要研究对象。将2020年11月至2021年1月进行的核酸采样标本(共10704例)作为对照组,采用常规核酸采样流程;将2021年2月至2021年4月进行核酸采样标本(共10541例)作为观察组,通过经过FMEA优化后的应用信息化手段核酸采样全过程控制,对已筛查出来的高危失效模型实施有针对性的整改。结果表明在经过FMEA优化后,观察组高危失效模型的RPN值显著下降,核酸标本丢失率均显著降低,标本扫码执行率、标本数量正确率提高(均P<0.05)。结论利用FMEA对核酸标本进行全程信息化安全管理,可以减少差错的发生,缓解医患矛盾,节约人力。

英文摘要:

Objective To explore the application and effect of failure mode and effects analysis (FMEA) in the whole process of nucleic acid sample sampling, counting, transporting and receiving. Methods the samples of nucleic acid sampling in a hospital from November 2020 to April 2021 were selected by convenient sampling method. The nucleic acid sampling samples (10704 cases in total) from November 2020 to January 2021 were taken as the control group, and the nucleic acid sampling process was adopted; The nucleic acid sampling samples (10541 cases in total) from February 2021 to April 2021 were taken as the observation group, and the whole process control of nucleic acid sampling by applying information-based means optimized by FMEA was adopted to carry out targeted rectification on the screened high-risk failure modes. Results after FMEA optimization, the RPN value of high-risk failure modes in the observation group decreased, the loss rate of nucleic acid samples decreased significantly, and the execution rate of sample scanning and the correct rate of sample number increased (all P < 0.05). Conclusion FMEA can reduce the occurrence of errors, alleviate the contradiction between doctors and patients, and save manpower.
