【】 背景 人口老龄化引起的老年跌倒问题日益严峻,不仅发生率和死亡率高,还可能导致老年人发生身心创伤,跌倒恐惧也因此逐渐备受关注。为有效预防老年人跌倒的再次发生并提升他们的生活质量,对跌倒恐惧进行及时评估与干预具有重要意义。目的 综合分析国内外跌倒恐惧评估工具,为评估老年人跌倒恐惧程度提供参考。方法 计算机检索PubMed、CINAHL、Web of Science、PsycINFO、Embase、Cochrane Library、维普数据库、中国知网、SinoMed、万方共10个数据库,检索时限为建库至2023年11月1日。由2名经过培训的研究者独立筛选文献并提取数据。采用COSMIN风险偏倚清单对纳入评估工具的测量学性能进行评价。结果 共检索14 816篇文献,最终纳入39篇文献,共计26种跌倒恐惧评估工具,评估对象分别为老年人(n=19)、照顾者(n=3)、其他人群(n=4)。COSMIN风险偏倚清单评价结果表明国际版跌倒效能量表(FES-I)、简明国际跌倒效能量表(short FES-I)、图像版跌倒效能量表(Icon-FES)、简短版图像跌倒效能量表(Icon-FES)、修订版跌倒恐惧问卷(FFQ-R)测量属性检验最多且测量学性能偏倚风险相对较低。结论 老年人跌倒恐惧评估工具方面的研究已较为系统和成熟,国际版跌倒效能量表(FES-I)在国际上应用较广泛。然而,针对照顾者跌倒恐惧评估工具的研究相对较少。未来学者可依照COSMIN风险偏倚清单,结合中国的文化特征与医疗背景,研发出适合中国老年人群及其照顾者的跌倒恐惧评估工具。 |

【】 Background The issue of increased falls among the elderly due to an aging population is both significant and pressing. With high incidence and mortality rate,falls may also cause the aged to get hurt and traumatized. For this reason,the fear of falling has been in the limelight in recent years. Timely assessment and intervention of the fear of falling can play a crucial role in effectively prevent the re-occurrence of falls and enhance the quality of life for old people.Objective This study aims to conduct a comprehensive analysis of tools used to assess the fear of falling,serving as reference for gauging fear levels.Methods A total of 10 electronic databases were systematically searched,including PubMed,CINAHL,Web of Science,PsycINFO,Embase,Cochrane Library,VIP Database,CNKI,SinoMed,and Wanfang . The search time frame is from the date since these databases established to November 1,2023. Two trained researchers screened literature and extract data. The COSMIN risk of bias checklist is adopted to appraise the methodological quality of the assessment tools included in the study.Results A total of 14 816 documents were searched,from which 39 were deemed pertinent,encompassing 26 fear of falling assessment tools. The subjects of these assessments were the elderly (n=19),caregivers (n=3),and other groups (n=4). The COSMIN risk of bias checklist inventory results revealed that the Falls Efficacy Scale-International (FES-I),the Short Falls Efficacy Scale International (short FES-I),the Iconographical Falls Efficacy Scale (Icon-FES),the Shortened Version of the Icon-FES (short Icon-FES),and Fear of Falling Questionnaire Revised could best examine the measurement properties and were deemed to have a relatively lower risk of bias in metrological performance.Conclusions The research into fear of falling assessment tools for the elderly is relatively advanced and methodical,and the Falls Efficacy Scale-International (FES-I) is extensively utilized worldwide. However,the research on assessment tools for caregivers’ fear of falling remains limited. Future studies could be guided by the COSMIN risk of bias checklist and combine China’s unique cultural characteristics and medical background to develop a fear of falling assessment tool that is appropriate for both Chinese elders and their caregivers. |
