      目的 研究体位调节对于预防俯卧位椎动脉内径变窄的效果,以及改善患者舒适度的意义。 方法 招募60位健康志愿者随机分为两组,均使用超声测量仰卧位时双侧椎动脉内径(VAID),作为正常值参考。实验组(A组)30位:取俯卧位,通过体位调节使双侧VAID尽可能接近正常值,记录VAID变化值(△D实验);对照组(B组)30位:同样取俯卧位,但不进行体位调节,测量双侧VAID,记录VAID变化值(△D对照)。两组在保持俯卧位2小时后,分别采用VAS及DHI测定头痛及头晕评分以评估两组患者的舒适度。数据统计使用SPASS 18.0统计软件。结果 实验组俯卧位时经过头部体位调节,VAID变化不明显(P=0.67),△D实验=0.078±0.028 mm;而对照组俯卧位时不进行头部体位调节,VAID变化明显(P<0.01),△D对照=0.910±0.175mm;在头痛及头晕评分评估中,实验组舒适度明显高于对照组(P<0.01)。结论 通过对健康受试者的研究,发现调整头部体位可以有效防止俯卧位VAID变窄,从而明显降低俯卧位术后头痛和眩晕的发生率,提高患者舒适度。
英文摘要:
      Objective To study the effect of position adjustment to prevent decreasing of vertebral artery"s inner diameter(VAID) on prone position, as well as its value to promote patients" comfort degree . Method 60?healthy volunteers were recruited and divided into two groups randomly. All volunteers" VAID on supine position were measured by ultrasound and used as normal figure reference. Experimental group (Group A); 30 volunteers" VAID were measured on prone position, position adjustments are performed to keep the figure of VAID on prone position as the same on supine position ones as possible. and the difference of VAID between on supine position and on prone position were also recorded as△Dexperimental. Control Group(Group B) 30 volunteers" VAID were also measured on prone position without any position adjustment, the difference of VAID between on supine position and on prone position were recorded as△Dcontrol. After maintaining the prone position 2 hours, all patients" comfort degree were evaluated by using VAS and DHI scale which are accepted as headache and dizziness evaluation methods worldwide. The statistics and analysis were performed by SPSS 18.0. Result After proper position adjustment, the figures of VAID between on supine position and on prone position in Group A got no difference statistically(P=0.67), and △Dexperimental=0.078±0.028 mm;But without position adjustment, the figures of VAID between on supine and prone position in Group B got obvious difference statistically(P<0.01), and △Dcontrol=0.910±0.175mm;The comfort degree in Group A is obvious higher statistically, comparing with Group B(P<0.01). Conclusion Base on the study with healthy volunteers, we could find that position adjustment could prevent the decrease of VAID on prone position effectively, decrease postoperative headache and dizziness, and improve patients" comfort degree.
