| 目的 探讨社区糖尿病健康管理小组在2型糖尿病患者中的应用。方法 选择2015年4月~10月上海市徐汇区斜土街道社区卫生服务中心卫生站所管辖社区糖尿病居民中抽取2型糖尿病患者600例。成立糖尿病健康管理小组,由12名全科家庭医生和12名家医助理组成,并从600例糖尿病患者中抽取12名志愿者组长,由全科家庭医生和家医助理对志愿者组长先进行培训,考核合格后,负责活动的组织。制定课程表,对600名糖尿病患者采用集体授课、课后讨论,实践操作指导强化胰岛素注射技术演练、上门指导、督促糖尿病患者自我计划制定与实施等方法进行糖尿病全程健康管理。教育前后采用自行设计的糖尿病相关知识问卷进行调查,观察患者管理前后糖尿病相关知识的变化及对患者糖代谢的影响。结果 进行糖尿病健康管理的患者糖尿病相关知识掌握情况和糖代谢水平明显优于教育前,差异有统计学意义(P均,＜0.01)。结论 糖尿病健康管理小组可明显促进糖尿病患者的自我管理,提高糖尿病患者对糖尿病的认识水平,可有效的控制血糖,对有效控制血糖起到积极地作用。 |

| 【】Objective to explore the community diabetes health management teams in the application of the patients with type 2 diabetes. Methods from April 2015 to October in Shanghai xuhui district XieTu street community health service center in diabetes health posts have jurisdiction community residents from 600 patients with type 2 diabetes. Diabetes health management team established, by 12 general family doctor and 12 famous medical assistant, and drawn from 600 patients with diabetes 12 volunteers group leader, the general family doctor and family medical assistant to volunteer team leader training first, after assessment is eligible, responsible for the organization of the activity. Formulates the curriculum in 600 patients with diabetes use collective teaching, after class discussion, practical guide intensive insulin injection technique drills, the door to guide and supervise and urge diabetes self plan formulation and implementation methods for diabetes health management all the way. Diabetes used before and after education knowledge questionnaire investigation, observation before and after the management of patients with diabetes related knowledge of change and the effects on glucose metabolism in patients with. Results diabetes health management of patients with diabetes related knowledge and sugar metabolism level education is obviously better than the former, the difference had statistical significance (P all, < 0.01). Conclusion diabetes health management team can obviously promote the self-management of the diabetic patient, improve the level of diabetes knowledge of diabetes, which can effectively control blood sugar, to play a role in effective control of blood sugar. |
