      目的：口腔肿瘤患者术后通常需要气管切开以建立安全的气道管理。本研究旨在比较人工鼻与传统的湿化器对气道湿化的效果以及气道其他各项情况。方法：选取收治入我院ICU病房的口腔肿瘤气管切开自主呼吸的患者120名随机分为人工鼻组和湿化器组各60名。随后对其气道湿化度、痰粘度、痰培养阳性率、肺部感染发生率及患者家属满意度进行评估和比较。结果：两组患者一般情况无差异,人工鼻组的气道湿化适中明显优于湿化器组(P=0.014)；且痰粘度正常的患者明显多于湿化器组(P<0.01),痰培养阳性率在第六及第七天人工鼻组显著少于湿化器组(P=0.031和P=0.024)而肺部感染发生率两组间无差异。患者家属满意度人工鼻组明显高于湿化器组(P=0.015)。结论：相较传统湿化器,人工鼻在口腔肿瘤气切患者术后气道的湿化及痰液的稀释方面有显著的优势,且能提升患者家属满意度,其临床应用值得推广。
英文摘要:
      Objective：Oral tumor patients usually need tracheotomy for safe airways after surgery. This study is to compare the traditional humidifier to the heat and moisture exchanger (HME) for their effect of airway humidification. Methods: 120 oral tumor patients were included in our study and divided randomly into two groups. We assess the airway humidification degree,viscosity of the sputum, positive rate of sputum culture, the occurrent rate of pulmonary infection and the patients relatives’ satisfaction. Results: There is no significant difference between groups in general condition. HME group is significantly better than traditional humidifier in airway humidification degree (P=0.014),viscosity of the sputum (P<0.01), positive rate of sputum culture in day 6 and day 7 (P=0.031and P=0.024) and the patients relatives’ satisfaction (P=0.015) but no difference in the occurrent rate of pulmonary infection. Conclusions: We concluded that, compared to traditional humidifier, the heat and moisture exchanger (HME) is better than traditional humidifier in airway management and increase the patients relatives’ satisfaction. Its clinical application is worth promoting.
