      目的: 探讨完善的术前、术后感染预防及护理策略，以减少泌尿系结石术后患者感染性休克的发生。方法: 将临床病人分为A，B两组。A组：2013年1月～2015年12月我院泌尿系结石微创手术治疗（输尿管硬镜、软镜及经皮肾镜碎石术等）的患者1050例。B组：2016年1月～2017年11月620例微创手术治疗的患者实施相应的预防治疗及护理措施。对两组患者的临床资料、治疗情况进行回顾性分析、对照。结果: 干预前A组1050例患者共发生感染性休克22例，发生率2.09%，死亡3例；干预后B组620例患者共发生感染性休克3例，发生率0.48%，无死亡病例。差异有统计学意义 (P<0.05)。结论: 泌尿系结石患者术后发生感染性休克与多种因素有关，术前根据尿常规及尿细菌培养结果合理的应用抗生素以及完善的术后相应护理措施能有效降低泌尿系结石术后感染性休克的发生率。
英文摘要:
      Object: To explore the preoperative and postoperative infection prevention and nursing strategies for patients with urinary calculi, in order to to reduce the incidence of septic shock after urinary calculi operation. Methods: Retrospective analysis of all patients received minimally invasive surgery (ureteroscopy, flexible ureterscopy and percutaneous nephrolithotomy) from 01/2013 to 11/2017. All patients were divided into two groups. Group A, 01/2013-12/2015, all patients haven’t received preventive treatment and nursing measures due to clinical operation reality. Group B, from 01/2016 to 11/2017, patients were treated with corresponding preventive treatment and nursing measures account for treatment progression. The clinical data and treatment measures of these two groups were analyzed retrospectively. Results: In all, 1670 patients were included. In group A (no intervention group, totally 1050 patients enrolled), there were 22 cases (2.09%)of septic shock and 3 cases of death (0.3%). In Group B (intervention group, totally 620 patients enrolled), there were 3 cases (0.48%)of septic shock occurred and no death. The difference was statistically significant (P<0.05). Conclusion: Postoperative septic shock in patients received urine test and bacterial culture results, use antibiotics reasonably and provide appropriate nursing measures after operation can effectively reduce the incidence of postoperative septic shock after urinary calculi.
