      摘要：目的 探究增大翻身角度是否有助于改善重症肺炎机械通气患者呼吸功能。方法 重症肺炎机械通气患者122例，随机分为对照组41例，治疗一组41例，治疗二组40例。分别采用30度、45度、60度的侧卧位-半坐卧位-侧卧位的护理方式。记录患者上机前2小时，上机后2小时、6小时的呼吸频率、氧合指数以及并发症发生情况。结果 上机后2小时三组患者呼吸频率、氧合指数差异有意义（P＜0. 05），上机后6小时三组患者呼吸频率无差异（P＞0. 05），氧合指数差异有意义（P＜0. 05）。治疗一组患者呼吸频率、氧合指数恢复最迅速。对照组和治疗二组均发生1例并发症，治疗一组未发生并发症，三组患者并发症差异无统计学意义（P＞0. 05）。结论 采取45度侧卧位护理方式对患者呼吸指标的改善最明显。

      Abstract: Objective Investigate whether increasing the angle of turning helps improve respiratory function in patients with severe pneumonia in mechanical ventilation. Method severe pneumonia patients 122 cases with mechanical ventilation were randomly divided into control group 41 cases, first treatment group 41 cases, second treatment group 40 cases. Take 30-degree, 45-degree, and 60-degree lateral position turn Semi-sitting and lying. The respiratory rate, oxygenation index, and complications were recorded 2 hours before and 2 hours after and 6 hours after the operation .Result The difference in respiratory frequency and oxygenation index between the three groups was significant at 2 hours after the operation（P＜0.05）, After 6 hours ,the difference in respiratory frequency between the three groups was no significant at after the operation（P＞0.05）but oxygenation index was significant（P＜0.05）, respiratory frequency and oxygenation index recovers most quickly in first treatment group。control group and second treatment group have one complications ,but first treatment group without complications, the difference between the three groups was no statistically significant（P＞0.05）. Conclusion The most obvious improvement of respiratory index was the patients of 45-degree lateral position .
