      目的 探讨急诊抢救室患者生命体征与营养风险的相关性,营养风险与患者预后的关系,营养支持治疗对患者营养状况及预后的作用。方法 采用NRS-2002量表对瑞金医院北院急诊抢救室136例患者进行营养风险评估;采用EWS评分评估患者生命体征;采用血清ALB评估营养支持治疗的效果。结果 急诊抢救室营养风险发生率为69.86%,营养支持比例为40%;入抢患者营养风险与入抢时EWS评分呈正相关(r=0.299,p=0.00);在不同营养风险患者中,临床结局构成比不同,差异有统计学意义(Z=-2.468,p=0.014);在营养风险患者中,给予营养支持治疗的患者较未予营养支持的患者ALB的下降程度小,差异有统计学意义(t=-3.208,p=0.002);同时,接受营养支持的患者较未接受营养支持的患者预后更好,差异有统计学意义(Z=-2.709,p=0.007)。结论 急诊抢救室营养风险发生率高,营养支持应用率低;入抢时患者生命体征、临床结局与营养风险相关,营养风险越大,预后越差;使用营养支持治疗可明显提高患者营养状况,并改善预后。

      Objective To discuss the connection between nutritional risk and vital signs and prognosis,as well as whether nutritional support improves prognosis.Methods Evaluate patients’ nutritional risk with NRS-2002 and compare the prognosis between nutritional risk and non-nutritional risk patients;Use EWS to indicate patients’ vital signs and ALB variation to indicate effect of nutritional support.Results Nutritional risk incidence in resuscitation room is 69.86% while nutritional support incidence 40%.The nutritional risk is correlative with vital signs .(r=0.299,p=0.00).The prognosis in nutritional risk and non-nutritional risk patients are different(Z=-2.468,p=0.014).The decline of ALB variation is slighter in nutritional support patients(t=-3.208,p=0.002)who obtain better prognosis(Z=-2.709,p=0.007).Conclusion Nutritional risk incidence in resuscitation room is high while nutritional support incidence is low.The nutritional risk is correlative with patients’ vital signs. The greater nutritional risk is,the worse the prognosis is.Nutritional support improves patients’ condition and prognosis.
