      目的:观察不同温度肠内营养液维持滴注对神经外科危重症患者腹胀、腹泻发生率的影响。方法:选取2017-10月至2019-03月我院神经外科住院需肠内营养液维持滴注的150例危重症患者,随机分为室温组、恒温器37~40℃组、恒温器28~30℃组,每组各 50 例,观察并记录鼻饲后7-14天内腹胀、腹泻发生的例数,实验结束后分别计算各组腹胀、腹泻发生率。结果:①. 室温组、37~40℃组、28~30℃组患者腹泻发生率分别为36%,10%,14%,三组腹泻发生率存在统计学差异(χ2=12.25,P=0.002);②. 室温组、37~40℃组、28~30℃组患者腹胀发生率分别为24%,6%,8%,三组腹胀发生率率存在统计学差异(χ2=8.799,P=0.012);③通过Bonferroni方法两两比较,其中室温组腹泻、腹胀发生率最高,37~40℃与28~30℃组间腹泻、腹胀发生率无统计学差异。结论:适当提高肠内营养液温度可有效降低神经外科危重病患者肠内营养液相关腹泻、腹胀发生率,肠内营养液温度保持28~30℃可满足要求,在某种程度上可简化护理操作,提高护士工作效率。

      Objective:To observe the effect of different temperature and enteral nutrition infusion on the incidence of abdominal distension and diarrhea in critically ill patients in neurosurgery.Methods:From October 2017 to March 2019, 150 critically ill patients requiring continuous infusion of enteral nutrition solution in our department of neurosurgery were randomly divided into room temperature group, thermostat 37~40℃ group and thermostat 28~30℃ group, 50 cases in each group. The number of cases of abdominal distension and diarrhea within 7-14 days after nasal feeding was observed and recorded. The incidence of abdominal distension and diarrhea in each group was calculated after the end of the experiment. Results:The incidence of diarrhea was 36%,10%,14% respectively among room temperature group, 37~40℃ group and 28~30℃ group, and there were statistical differences in the incidence of diarrhea anong the three groups(χ2=12.25,P=0.002); The incidence of abdominal distension was 24%,6%,8% respectively among room temperature group, 37~40℃ group and 28~30℃ group, and there were statistical differences in the incidence of abdominal distension anong the three groups(χ2=8.799,P=0.012);Comparisons were made by Bonferroni methed, in which the highest incidence of diarrhea and abdominal distension occurred in the room temperature group, but there was no significant difference between 37~40℃ group and 28~30℃ group. Conclusion:Increasing the temperature of enteral nutrition solution properly can effectively reduce the diarrhea and abdominal distension associated with enteral nutrition solution in critically ill patients in neurosurgery. Maintaining enteral nutrition solution at 28~30℃ can be suitable for clinical practice, to some extent which can simplify nursing operation and improve work efficiency.
