中文摘要:
      目的 基于老年人健康状态评估下的照护实践，旨在为医养结合照护服务体系制定的标准和完善提供客观依据。方法 使用COOP/WONCA功能状态量表，对上海某敬老院342例住养老人健康评估，分析生命质量状况与照护等级关系。结果 老年人共病现象明显的基础上整体健康评价偏低；不同照护等级对总体生命质量和生理适应性、情感、日常活动、社会活动、健康变化、整体健康、疼痛各个维度均有统计学差异的影响（P<0.05）。结论 建立和完善系统规范的医养照护服务体系，且纳入社区卫生服务中心的功能定位,持续提高照护医疗专业化、多元化的综合能力，对健康老龄化的作用举足轻重和迫在眉睫。
英文摘要:
      Objective Based on the nursing practice under the assessment of the health status of the .elderly,The aim is to provide objective basis for the standard and improvement of the medical and nursing service system.Methods Using the COOP/WONCA functional status scale, Health assessment of 342 elderly people living in a nursing home in Shanghai, The relationship between life quality and care level was analyzed.Reults Elderly comorbidity is evident on the basis of low overall health evaluation,Different nursing grades had statistically different effects on overall quality of life and physiological adaptability,emotional daily activities,social activities,health changes,overall health and pain（P<0.05）.Conclusion To establish and improve a systematic and standardized medical and nursing care service system, And include the functional positioning of community health service centers, Continue to improve the comprehensive capacity of specialized and diversified care medical care,The role of healthy aging is very important and urgent.
