      目的:探讨在新型冠状病毒肺炎(COVID-19)疫情期间内镜中心开展胃肠道诊疗工作的必要性和安全性。 方法:针对COVID-19流行期间调整流程,防护物资和人力资源管理,创建保护性物理隔离屏障,一线医护人员行抗体检测,患者及家属通过电话随访确认是否感染新冠肺炎。同时采用回顾队列研究方法,分析2020年2月20日至4月30日,来本内镜中心行门诊胃肠道诊疗的病例资料(疫情期间诊疗组)与2019年同期作为对照组,比较疫情期间胃肠道检查的总体情况以及消化道恶性肿瘤检出情况。 结果:疫情期间,COVID-19血液IgM/IgG抗体检测,未发现医护人员感染,电话随访未发现患者检查后及家属感染新冠肺炎;疫情期间胃肠道的诊疗量是对照组的53.5%,恶性肿瘤检出率较对照组明显升高,上消化道恶性肿瘤疫情期间诊疗组4.58%,对照组1.22%,结直肠恶性肿瘤疫情期间诊疗组7.45%,对照组3.59%。 结论 在COVID-19的流行期,严格遵循指南解读与规范的标准前提下,做好相对隔离的防护护理干预措施,保障了尽早开展内镜临床诊疗工作的安全性,与2019年同比,疫情期间,胃肠道检查量锐减,但消化道恶性肿瘤发现率显著升高,提示尽早复工的必要性。

      Aim:To explore the necessity and safety of continuing endoscopic procedures during COVID-19 pandemic. Methods: Workflow, protection supplies and staff management were adjusted according to COVID-19 pandemic conditions. Physical protection shields were established. COVID-19 antibodies were tested in first-line medical staff. Patients and relatives were confirmed with epidemiological survey through telephone follow-up. Meanwhile, a retrospective cohort study was performed including patients seeking out-patient digestive endoscopic procedures from Feb 20, 2020 to Apr 30,2020 as the exposure group and patients during the same period in 2019 as the control group. Detection rate of malignancies was compared between the two groups. Results: Serum COVID-19 IgM/IgG test during the pandemic revealed out that no medical staff were infected. Telephone follow-up did not find COVID-19 infections among patients and relatives after endoscopic procedures. During the pandemic, the total volume of out-patient endoscopic procedures was only 53.5% of the total volume during the same period in 2019, however, the detection rate of malignancy was significantly higher, with 4.58% v.s. 1.22% in upper gastrointestinal tract and 7.45% v.s. 3.59% in colon and rectum.[1.复旦大学附属中山医院内镜中心 上海200032 2.复旦大学附属中山医院门诊办公室 上海200032 基金项目:1.上海市科学委员会资助项目(164119504050)2.上海市护理学会资助项目(2016MS-B02) ] Conclusions: During COVID-19 pandemic, strict adherence to guidelines and regulations as well as relatively isolative protection rules could ensure the safety of early resumption of endoscopic procedures. Compared to the same period in 2019, the total volume of out-patient endoscopic procedures decreased sharply with significantly increased detection rate of malignancy, indicating the necessity of early resumption of endoscopic procedures.
