| 摘要：目的 观察乌针漱口液口腔护理预防ICU患者呼吸机相关性肺炎（VAP）的防治效果。方法 选取2019年4月至2020年6月在广西中医药大学附属瑞康医院ICU收治的92例经口气管插管机械通气治疗的患者,采用随机数字法分为观察组和对照组，每组46例。观察组采用乌针漱口液进行口腔护理，对照组采用生理盐水进行口腔护理，比较两组患者护理后的临床肺部感染评分(CPIS)、机械通气时间、口腔评估指南（0AG）评分、口咽及气管内定植菌发生率、VAP发生率、满意度的情况。结果 护理后，观察组的CPIS评分、机械通气时间均明显低于对照组，差异有统计学意义(P＜0.05)；观察组的口腔黏膜、牙龈、唾液、口腔异味、吞咽功能和0AG总分明显均优于对照组，差异有统计学意义(P＜0.05)；观察组的口咽及气管内定植菌发生率、VAP发生率均明显低于对照组，差异有统计学意义(P＜0.05)；观察组口满意度明显高于对照组，差异有统计学意义(P＜0.05)。结论 乌针漱口液口腔护理能更好地降低ICU机械通气患者VAP的发生率，减少机械通气时间，改善口腔健康情况，患者满意度较高。 |

| Abstract: Objective To observe the the preventive and curative effects on oral care of Wuzhen mouthwash for preventing ventilator-associated pneumonia(VAP) in ICU patients. Methods From April 2019 to June 2020, 92 cases with invasive mechanical intubation in ICU after surgery from Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine were selected, and divided into the observation group (n=46) and the control group (n=46) according to therandom number table method. The patients in the control group were given oral care with Wuzhen mouthwash and the patients in control group were given oral care with regular oral cavity nursing. Clinical pulmonary infection score (CPIS) after nursing, mechanical ventilation time, oral assessment guide (0AG) score, incidence of colonization in oropharynx and trachea, incidence of VAP, and satisfaction were compared. Results After nursing, the CPIS score and mechanical ventilation time in the observation group were significantly lower than those in the control group, with statistically significant differences(P＜0.05). After nursing, the oral mucosa, gingiva, bad breath, saliva, swallowing function and total score of 0AG in the observation group were significantly better than those in the control group, with statistically significant differences(P＜0.05). After nursing, the incidence of colonization in oropharynx, trachea and VAP in the observation group were significantly lower than those in the control group, with statistically significant differences(P＜0.05). After nursing, the satisfaction in the observation group were significantly higher than those in the control group, with statistically significant differences(P＜0.05).Conclusion The oral care of Wuzhen mouthwash can better reduce the incidence of VAP in ICU patients under the condition of mechanical ventilation, decrease mechanical ventilation time, improve oral health and satisfaction of patients. |
