      目的:改变传统血透室血源性传染病标志物的监测模式,探讨应用“血液透析感控预警系统”在血透室院内感染实时监测中的效果。方法:分别使用“手工查阅”法及“血液透析感控预警系统”对198例上海市交通大学医学院附属仁济医院血透室门诊维持性血液透析(maintenance hemodialysis,MHD)患者进行血源性传染病标志物的监测,记录监测所用时间,计算实查率及准确率,采用配对样本t检验,X2检验对比分析2种监测方法的差别。结果:应用“血液透析感控预警系统”方法后,监测所用时间明显短于“手工查阅”法,差异有统计学意义(t=184.482,P=0.000);实查率由95.5%升至99%,差异有统计学意义(x2=4.582,P=0.032);准确率由89.39%升至99.49%,差异有统计学意义(x2=19.251,P=0.000)。结论:运用“血液透析感控预警系统”能够快速高效查阅MHD患者的血源性传染病标志物,提高实查率及准确率,达到实时预警目的,同时该系统能满足多部门同时查阅,实现了三级无缝隙监控。

      Objective: To change the traditional mode for monitoring the indicators of blood-borne infectious diseases in hemodialysis and explore the effects of applying " early warning system for hemodialysis infection control " in real-time monitoring of nosocomial infection. Methods: Infectious disease indicators were monitored in 198 outpatients with maintenance hemodialysis(maintenance hemodialysis, MHD) at Renji Hospital Affiliated to Medical College of Shanghai Jiaotong University by "manual inspection" and " early warning system for hemodialysis infection control ", respectively. The monitoring time was recorded; the actual checking rate and accuracy rate were calculated; and the differences between the two methods were compared and analyzed by paired sample t test and x2 test. Results: After applying " early warning system for hemodialysis infection control ", the monitoring time was significantly shorter than that of "manual inspection", and the difference was statistically significant(t=184.482,P=0.000);The actual checking rate increased from 95.5% to 99%, with statistically significant difference(x2=4.582,P=0.032).The accuracy rate increased from 89.39% to 99.49%, and the difference was statistically significant(x2=19.251,P=0.000).Conclusion: The application of "early warning system for hemodialysis infection control" can check the indicators of blood-borne infectious disease in MHD patients quickly and efficiently, improve the actual check rate and accuracy rate, and achieve the purpose of real-time early warning. This system can achieve simultaneous check by multiple departments, which makes three-level seamless monitoring possible.
